Initial Location Assessment Form
The Initial Location Assessment Form only needs to be completed once and returned to Milwaukee Riverkeeper in person, by mail, or by scanning and emailing to firstname.lastname@example.org. If anything indicated on the form drastically changes over time, please complete and return another Initial Location Assessment.
Cleanup Report Form
Please fill out a Cleanup Report Form for each of your cleanups and return it to Milwaukee Riverkeeper in person, by mail, or by scanning and emailing the form to email@example.com within one week of your cleanup.

Full-Length Liability Waiver
All volunteers must read and sign a liability waiver before participating in a river cleanup. Volunteers under the age of 18 must have a parent or guardian sign a waiver on their behalf. Milwaukee Riverkeeper’s Short-Form Liability Waivers (below) help save paper in large volunteer groups!
Short-Form Liability Waiver
These sign-in sheets save paper and may be used in place ofthe full-sheet waiver as long as all volunteers still read the full-length waiver.
